Any benefits eligible UALR employee who participates in any of the below activities can share in the Wellness Pays money pool. Completion the Personal Health Appraisal is the entry point into Wellness Pays. Participants can earn Trojan Points by participating in any or all of the activities listed below. At the end of the fiscal year a cash incentive pool will be divided between the participants. The more Tojan points a participant earns the greater the percentage of the money pool given to the participant. The maximum amount of money earned by one individual is $100. Watch on facfocus and staffocus for details of reporting your points.
Example: You fill out the PHA, average 4 days a week of Aerobic Activity, participate in EDoc and Step into Spring. You earn 35pts.
Fill out the worksheet below and mail to Susan Meador at the end of the fiscal year. (you can select the chart and print it off) For each activity you participate include details about your participation. Example: tell us which LESC course you attended, which workshop, which exam, which road race.
