We send application forms and application instructions to every secondary school science chairperson and math chairperson in Arkansas on January 2nd. If you cannot obtain the forms at your school, you can download the forms at the bottom of this page. You also can send us an e-mail (jdwinter@ualr.edu) and we will send you the forms electronically.
This page contains links for application forms for the 2008 Arkansas STRIVE program. See the bottom of this page for instructions on downloading and printing the forms. You should also consult the Instructions for Applying section of our website for information on completing the form.

The documents will automatically download on Macintosh and most PC computers.
On some computers, the files will open and you should use the "Save As" command to download to your computer.
We recommend that you use the electronic application form (applicatE.doc) because you can easily type on this form using your computer and you will be able to send us a neat copy. Moreover, you can send this copy quickly back to us by attaching it to an e-mail message addressed to jdwinter@ualr.edu or you can send it back by mail. This document can be opened most easily by Microsoft Word. If you send the form back by e-mail, be sure to send copies of your transcripts and teaching certificates by mail.
The other application forms can be printed out and distributed to other teachers, or filled out by hand (typing is preferred).
